Complex Tooth Extraction

Modern dentistry has achieved remarkable breakthroughs in recent decades. However, in some cases, tooth extraction is the only solution. Two types of extractions can be performed. Simple tooth extractions do not typically cause complications. It takes a few days for the gum tissue to heal, and stitches are not necessary.

A complex dental extraction involves surgical intervention. The procedure is more complicated, requiring more time and preparation.

Indications for a Complex Tooth Extraction

• the wisdom tooth has not fully erupted or completely hidden underneath the gum
• a wisdom tooth is misaligned or embedded in the cheek
• molars with multiple roots (more than 3)
• the root of a tooth is severely curved, bent inward, or damaged
• the root of a tooth had become fused to the jaw bone
• severe inflammatory process, cyst or fistula
• heavily damaged or decayed tooth
• a tooth with a large dental filling may not withstand the pressure of the forceps during removal

Preparing for a Complex Tooth Extraction

Before performing an extraction, Dr. Joe Morabit conducts a consultation which includes a thorough oral checkup and dental X-ray examination of the tooth that has to be removed. This is an important stage for the outcome of the surgery since it largely depends on the accuracy of the diagnosis. According to diagnostic results, Dr. Joe evaluates the situation, assessing the number, size, and shape of the roots, as well as their relationship with the mandibular canal and the maxillary sinus floor. Based on these data, he creates a surgical treatment plan and selects surgical instruments. If the patient has inflammation in the oral cavity, antibiotics should be given within 24 hours before the scheduled procedure.

It is critical for people with diabetes to inform the doctor about their condition. In this case, the doctor will check blood glucose levels before and after the operation. This is of particular importance since diabetic patients have a higher risk of inflammation and slower wound healing. Performing tooth extraction on patients with high blood pressure requires similar preparations. If blood pressure increases significantly after the surgery, it should be lowered immediately with medication.

To make sure healing progresses smoothly, patients with diabetes and hypertension should visit the doctor 1-2 times to monitor the process.

Recovery after a Complex Tooth Extraction

After a tooth is removed, it leaves a hole that will be gradually filled in with the new tissue.

Stages of Gum Healing

• formation of a blood clot (within 2-4 hours after extraction)
• transformation of the blood clot into granulation tissue (after 3-4 days)
• bone formation (after 1-2 weeks)

Recovery from wisdom tooth removal can take up to a month. This is due to the unusual structure of the root system that is atypical in shape and size. In fact, wisdom tooth removal is the most traumatic dental procedure. Sometimes tooth roots cannot be completely removed.

The total time for gums to be healed after tooth extraction will depend on the extent of tissue damage caused by surgery. If the procedure was complicated and required dental sutures, the pain may last up to a week. Sutures are usually removed no later than 7 days.

Side Effects After a Complex Tooth Extraction

• Pain

One of the normal side effects following a tooth extraction is pain. When the anesthesia wears off, you may experience painful sensations in the area of the extracted tooth. It can cause intense pain. In this case, dentists recommend taking painkillers.

It is normal to have an aching, nagging, or throbbing type of pain. The pain should steadily decrease within 2-4 days.

• Bleeding

• Swelling

This may occur if the patient accidentally damages the blood clot that formed at the site of tooth extraction after the surgery. • Fever

• Avoid sticky or chewy foods.
• Eat soft foods that won't irritate the wound.
• Do not take any medications that could interfere with normal blood clotting.
• Refrain from smoking and drinking alcohol for a while.
• Brush your teeth carefully.
• Do not rinse your mouth after the tooth extraction procedure. It can disturb the formation of a blood clot or damage the already formed one.

A root canal is a hollow section within the root of a tooth. It contains the pulp with its bundle of blood vessels and nerves. The tooth receives nutrition through the blood vessels.
